This might involve the loss of a limb, a grievous internal injury imposing specific penalties, senses being permanently damaged, or a mortal wound that puts the character on death’s door regardless of remaining Hit Points. These moments are handled narratively, focusing on the specific event and its immediate, dramatic impact rather than relying solely on abstract numbers. The goal is not arbitrary punishment, but to reflect the severe consequences of specific, dangerous situations within the ongoing story.
The aftermath of such critical injuries becomes a significant part of the character’s journey. While prompt and successful application of First Aid or Medicine might stabilize a character or prevent immediate death, reversing catastrophic physical damage often requires more. Proper roleplaying, depicting the struggle and recovery, is encouraged and can influence outcomes. Depending on the nature of the injury and the resources available, lasting consequences might remain even after mundane healing is complete—a lost eye impacting perception, a missing hand affecting manipulation skills, or chronic pain imposing occasional penalties. Overcoming such lasting trauma might necessitate seeking extraordinary measures, such as rare and powerful magical healing, divine intervention, or undertaking perilous quests to find legendary remedies, depending entirely on the possibilities available within your specific campaign setting.
